Advancing Health Equity: The Lakewood Health System Approach, Presented by Amy Wiese, MHA

This session will provide dietitians with an overview of the role and evolution of Lakewood’s Community Health Team and its impact on patients, families, and the broader community. Participants will learn about key programs and services that address health equity, social determinants of health, and barriers to care. The session will highlight opportunities for collaboration between dietitians and community partners, outline referral pathways and communication processes, and share real case examples that demonstrate effective interdisciplinary teamwork. This Lunch & Learn will equip dietitians with practical tools to better connect patients with the resources and support they need in their communities.
